ORGANISING AND IMPROVING PATIENT-CENTRED NURSING CARE DURING REHABILITATION

This article analyses the theoretical and practical aspects of organising patient-centred nursing care during rehabilitation. Based on a targeted literature review, a six-stage model is proposed, consisting of functional needs assessment, agreeing goals with the patient, individual planning, implementation of care, monitoring, and coordination of continuing care. The article addresses education that is understandable to patients and caregivers, interprofessional collaboration, and safety. The outputs comprise a structured account of organisational factors, a ten-item audit checklist, and criteria for separately assessing care processes, functional status, and patient experience. The article presents a scientific and methodological proposal rather than a report of a primary clinical experiment. The effectiveness of the model and the measurement properties of the audit tool need to be examined in future using real clinical data.
